Retirement or Separation

What happens to your benefits at separation?

  • Medical, Dental, Vision

    When you leave Ulta Beauty, your benefits will end at the end of the current pay period.

    You might be eligible to extend your medical, dental, vision, and/or Healthcare Flexible Spending account for up to 18 months beyond your separation date under COBRA. Information regarding options, prices and how to elect will be sent directly to your home address from HealthEquity WageWorks.

    You may also elect coverage through the Healthcare Marketplace or another group employer plan (e.g., a spouse’s plan, if available). If you choose this option, you must act right away after losing an employer’s healthcare coverage as there are enrollment deadlines you will have to meet.

    A Certificate of Creditable coverage provides important information about your Ulta medical coverage dates which you may need to provide if you apply for coverage elsewhere, such as a spouse’s plan or the healthcare marketplace.   • Health Savings Account

    If you are enrolled in the Health Savings Account, it is yours to keep and will continue to be managed by HealthEquity.

    Contact HealthEquity for more information on how to manage your account after leaving Ulta Beauty.

  • Flexible Spending Accounts

    The healthcare, dependent care, and limited Flexible Spending Accounts end as of your date of separation. Eligible expenses must be incurred prior to your last day of employment to be eligible for reimbursement. You have until November 30 of the following year to submit them for reimbursement (90 days following the end of Ulta’s benefit plan year). Any funds not spent will be forfeited. Forms and instructions can be found on HealthEquity’s website.

  • Life Insurance

    Your life insurance is able to be converted or ported to an individual policy directly with Unum. A packet with rates and more information will be sent to your home address following your separation. If you have questions, contact Unum at 800-445-0402.

  • MetLife Supplemental Plans

    Any MetLife Plan (including home insurance, auto insurance, pet insurance, life insurance, hospital, accident, critical illness, or pre-paid legal) can be continued, but you will need to contact MetLife to arrange an alternate payment plan. Contact MetLife directly at 888-343-6891.

  • Identity Theft

    If you have Identity Theft Protection through Aura and would like to continue coverage, contact Aura Identity Guard at 855-443-6891 to set up direct billing.

  • Time Off

    Any accrued but unused vacation time will be paid to you on your last paycheck.

    Sick and personal days are not paid out, unless otherwise required by state law.

  • 401(k)

    You can leave your money with Fidelity or choose to take the money and roll it into another plan. You can discuss your options and request a distribution directly with Fidelity at 800-835-5097 or www.401k.com.

  • Medicare

    Medicare is the federal government health insurance program that provides health care coverage. It is funded by Social Security and Medicare taxes you pay on your income. There are four parts to Medicare: Part A, Part B, Part C, and Part D.
