Eligibility

Full-time associates

Full-time associates are eligible for full-time benefits and are expected to work 30 (+) hours a week to maintain full-time employment status.

Part-time associates

Part-time associates are eligible for part-time benefits and are expected to work less than 30 hours a week.

Determining eligibility

Every 12 months, in compliance with the Affordable Care Act (ACA), hours are measured to determine full-time status based on average hours worked from June to June for benefits effective September 1. Associates who are considered full time will be placed in a stability period for the next 12 months during which they must be offered full-time benefits, regardless of hours worked during the stability period.

Every four weeks, in compliance with the Hawaii Prepaid Health Care Act, hours are measured to determine Hawaii associates who work 20(+) hours per week for four consecutive weeks and are offered medical coverage as a result.

Every 6 months (weeks 1-26 and 27-52), in compliance with the Ulta Beauty FT-PT Employment Status Policy, hours are measured to ensure full-time associates are working full-time hours. Associates who are considered full-time but average less than 30 hours in those measurement periods may be moved to part-time status and, as a result, lose benefits eligibility.

Benefits effective date

Your benefits are effective on the first day of employment, or movement to full-time or part-time status. This means you have 30 days from your date of hire or employment status change (not first day scheduled or in-office) to make benefit elections. If you are rehired within 30 days of your separation, your health benefits will be reinstated.

Associates moving from FT to PT who are not in a stability period and were previously enrolled in BCBS dental and VSP vision will be automatically enrolled in MetLife's dental and vision benefits. If you do not want these benefits, it is your responsibly to login to ultabenefits.bswift.com and waive them.

Dependent eligibility

Eligible dependents include:

  • Legally recognized spouse;

  • Domestic partner; and/or

  • Your child under age 26 (a dependent child may remain eligible beyond age 26 if they are disabled).
