IMPORTANT HEALTH COVERAGE TAX DOCUMENT
Your 1095-C form, which provides important health coverage information for your tax returns, is available each year by January 31st for the prior tax year. These are available in bswift effective 1/31/2026 forward.
Where to Find Your 1095-C
Bswift online enrollment portal: You can view and download your 1095-C form directly from bswift’s enrollment portal at ultabenefits.bswfit.com and clicking on the Manage/Upload Documents tile.
Request a Copy: If you prefer to receive a physical copy of your 1095-C form, you can submit a request through Benefits or the Associate Care and Support team.

Ulta Beauty associates can find Plan Documents and compliance documents like Summary Plan Descriptions (SPD), Summary of Benefits and Coverage (SBC), CHIP notices, Medicare Part D, and more by logging into bswift, your personalized benefits portal at ultabenefits.bswift.com and clicking on the Resource Library. You may also email benefits@ulta.com to request a paper copy.
Where to Find Your BCBS Medical Booklets
Ulta Beauty associates can find BCBS Medical Booklets by logging into their Blue Access for Members account at bcbsil.com.
