Leave of Absence and Accommodations
Associates might be eligible to take a leave of absence from Ulta Beauty under certain circumstances, including:
Having or adopting a child
A medical condition or injury
Caring for family member
Serving in the military
Other approved circumstances
To request a leave of absence, contact UNUM online at https://portal.unum.com or by phone at 866-868-6737.
Associates can also request a reasonable accommodation to perform the essential functions of their job online at adaassist.unum.com.

Pay While on Leave
Leaves are generally unpaid. If the leave is due to an illness or injury, including pregnancy and childbirth, disability benefits may be provided.
Disability benefits vary depending on employment status, the reason for the leave, and state disability programs. See the Disability page for more details on disability benefits.
Paid time off can also be used while on an approved leave of absence. See the Time Off section for more details.

Maternity Leave
Ulta Beauty’s Maternity Leave Policy provides all associates employed in the United States the ability to take up to 12 weeks of leave following the birth or adoption of a child.
In addition to paid time off and disability benefits, Ulta Beauty also has Parental Leave Pay for new parents to spend time away from work to bond with their child. Eligibility rules apply. Please See Ulta Beauty’s Maternity Policy and the Parental Leave Policy on the UltaNet > Policies page.
Many states have additional paid family leave time or disability pay that is applied first and may extend your time away and provide additional income while you are on leave.
These benefits coordinate with Ulta Beauty’s maternity leave, disability pay, and parental leave pay. It is your responsibility to apply for the job protection and benefits directly with the state and keep the Leave of Absence team notified of your approval. See the Policies tab on the UltaNet for more state-specific paid family leave information.

Return to Work
If a leave extension is needed, contact UNUM immediately to inform them of the additional time requested.
If the leave is for an associate’s own medical condition, a release from a physician is required before returning to work

Accommodations
Accommodations are changes or modifications made for people with disabilities so that they are able to perform their essential functions of their job.
Ulta Beauty will consider accommodation requests for, and as appropriate, provide reasonable accommodations during the application process, as well as our associates once hired, unless doing so would cause an undue hardship.
