Livongo for Diabetes and Hypertension Care
Livongo is a program available to Aetna and Horizon members, which provides eligible members with Livongo supplies, digital technology and coaching support for those with diabetes or high blood pressure. Through Livongo’s personalized and powerful program, those managing diabetes and/or hypertension receive all they need to feel in control of their health. Livongo provides digitally enabled tools, testing kits as needed, daily trackers and motivational coaching and guidance.
