The program provides a holistic approach to members with Diabetes through interventions such as Blood Glucose Monitoring, Medication Adherence & Optimization, Screenings and Lifestyle & Comorbidity Management. CVS Health has a team of Certified Diabetes Care Nurses to provide personalized coaching, monitoring, and one-on-one support.
Access to a Bluetooth-enabled glucometer (i.e. One Touch) that connects to the TDC mobile app OR a BioTel Care connected glucometer for high risk members.
Two comprehensive Diabetes Monitoring visits at any MinuteClinic at no out-of-pocket cost, including point-of-care A1C testing.
$0 copays for formulary glucose meter test strips and lancets delivered to your home at no out-of-pocket cost. (Note: diabetic medications must be filled through a CVS retail pharmacy or mail order).
Telephonic proactive outreach from Registered Nurses who are also Certified Diabetes Care and Education Specialists (CDCES) to provide coaching and guide members in closing gaps in care related to management of diabetes and related comorbidities.
Outreach from CVS Pharmacists (for CVS fillers) and HealthHUB Care Concierge to provide support to members related to their diabetes management.
Nutrition coaching from Registered Dietitians.
