How the Vision Plans Work
La-Z-Boy offers two Vision Plan options: The Basic Plan and the Buy-Up Plan.
Both plans offer:
Comprehensive coverage – from preventive eye care to eyeglasses and contact lenses.
Nationwide network of providers (including Walmart and Sam’s Club); you pay less when using in-network providers.
Same allowances for certain other services.
Discounts on laser vision correction, hearing exams, and hearing aids.
How the Plan Options Differ
Core Plan Buy-Up Plan Lowest payroll deduction Higher payroll deduction $10 copay for vision exams and standard lenses No copay for vision exams and standard lenses Frames are covered every 24 months Frames are covered every 12 months Good choice if you mostly need routine exams and have generally healthy eyes and good vision (no need for glasses or contact lenses). Good choice if you expect more vision care beyond routine exams and/or if you or your dependents need glasses or contact lenses.
