Short-term Disability

You are automatically enrolled in the short-term disability (STD) program through Lincoln Financial when you become eligible for benefits. The Company pays the full cost of this benefit.

  • If you suffer a disability from a qualifying non-occupational illness or injury while covered by this program, you will be eligible to receive weekly income benefits designed to help you replace your lost wages.

  • Before collecting benefits, you must satisfy an elimination period of seven calendar days following your date of disability.

  • Your absence may also qualify for protection under the Family and Medical Leave Act (FMLA). Information will be sent to you from Lincoln Financial regarding your rights and responsibilities under FMLA, which runs concurrently with your STD leave of absence.

  • Benefits may be paid for up to 26 weeks in a 52-week period if your disability is medically certified and your claim is approved by Lincoln Financial.

  • Reporting a Claim

    You may report a claim up to 30 days in advance of a planned disability absence (such as childbirth or prescheduled surgery) or as soon as you think your absence from work (due to a non-occupational injury or illness) may extend beyond seven days. If you have an unexpected illness or injury, report your claim as soon as possible.

    Note: This does not apply to teammates working in California and Rhode Island who are required to file for Temporary Disability Income through their respective states.

    1. Report your absence to your manager.

    2. Contact Lincoln Financial. Call the toll-free number at 1-888-729-1047 or submit your claim on the Lincoln Financial portal.

    3. You will be required to establish a username, password and security question if reporting on the Lincoln Financial portal (DICK’S Sporting Goods company code is DSGTM).

    4. Print, sign and date a copy of the Authorization to Release Information Form and leave with your physician or medical care provider.

    5. See your physician or medical care provider. Lincoln Financial requires your physician to provide information about your medical condition. If this information cannot be obtained, benefits may be delayed.

    6. You will need to provide:

      • Your name and Social Security number (SSN)

      • Your complete address and phone number

      • Your physician's or medical care provider's name and phone number

      • Your last day worked and first day absent from work because of the medical condition

      • Verification that you are employed by the Company

    You will also be asked additional questions about yourself, your physician or medical care provider and your medical condition.

  • Claim Verification

    When you file your claim, you'll receive your claim number and will be assigned a Lincoln Financial claims specialist. Have a pen and paper ready. You can also view the status of your claim on the Lincoln Financial portal. You will need your claim number.
