Long-term Disability

After 26 weeks (180 days) of medical leave, you may qualify for extended benefits through the long-term disability (LTD) program. Benefits under the LTD program replace a portion of your monthly covered earnings for the duration of your medically certified/approved disability, or until age 65.

Proof of good health for full-time hourly teammates

Note: You must be enrolled in the Voluntary LTD plan to be eligible for the benefit.

If you choose to enroll outside of the specified periods listed below, you must provide proof of good health (also known as evidence of insurability, or EOI). Lincoln Financial will send an Evidence of Insurability Request Form to you, if applicable.

Proof of good health is not required if you are:

• Enrolling within 31 days of your initial eligibility

• Enrolling within 31 days of a life/employment status change
