IMPORTANT HEALTH COVERAGE TAX DOCUMENTS
1095-C forms are not required to be mailed for 2025, for most states. Those teammates who were eligible for medical coverage and living in California, New Jersey, Rhode Island and Washington, D.C. will receive a 1095-C by mail at the home address on file in Workday.
All current and former teammates who were eligible for medical coverage in 2025 can access their forms online by following the instructions below:
Log in or register on mytaxform.com using the employer code 12164.
Teammates must provide verification of identity when creating a profile.
Locate the “Online Delivery Consent” section on the righthand side of the homepage.
Select the 1095-C box to receive the form electronically and submit your consent.
You can also request a copy of your 1095-C to be emailed or mailed to you by requesting it from the Benefits Department:

Summary of Benefits and Coverage (SBC)
