Medical

In 2027, Foot Locker will be joining the DICK’S Sporting Goods benefits program. DICK’S offers four PPO medical plans with Cigna. Here you will find highlights and key considerations. For detailed information about services and coverage, review the 2027 medical plans summary.

  • Medical Options

Four medical plans are offered, including:

    • Traditional PPO (Preferred Provider Organization) Plan

    • Core HRA (Health Reimbursement Account) Plan

    • Buy-Up HSA (Health Savings Account) Plan

    • Base HSA (Health Savings Account) Plan

These plans have several things in common:

    • They are all PPO (Preferred Provider Organization) plans. That means their network of doctors, hospitals and other providers has agreed to provide care at reduced rates. You always pay less by using network providers.

    • You are in charge of your care. You don’t need referrals to see specialists—you decide which provider to use each time you need care.

    • They pay 100% of the cost of in-network preventive care, including annual physical checkups, women’s preventive health services, recommended screenings and immunizations based on your age and frequency recommendations.

    • They have an out-of-pocket maximum to protect you from high-cost claims.

    • Includes prescription drug benefits.

    • No lifetime limits on benefits.

    • You will receive three member ID cards for Cigna medical, Evernorth Express Scripts prescription drug coverage and Lantern Health Center of Excellence program.

    • Telemedicine through MDLive is covered at 100%.

    Key differences:

Core HRA, Traditional PPO and Base HSA plans:

    • EMBEDDED DEDUCTIBLE: For plans with an embedded deductible, if you have an individual coverage tier, you pay coinsurance up to the individual out-of-pocket maximum once you satisfy the individual deductible. Once the individual out-of-pocket is reached, the plan pays at 100%.

For any tier other than individual (e.g., teammate & spouse, teammate & child(ren}, or family), once you or a covered member of the family reaches the individual deductible, that individual will pay coinsurance until the individual out-of-pocket is met and then the plan pays at 100% for that individual.

The remaining covered family members as a whole, or one covered family member, could satisfy the remainder of the family deductible. Once the family deductible is met, coinsurance will apply up to the family out-of-pocket maximum before the plan pays at 100%. For example, if you go into the hospital on day one and have a large hospital bill, you could satisfy the individual deductible and then pay coinsurance up to the individual out-of-pocket maximum. The remainder of the family would then have to satisfy the rest of the family deductible (it could be one covered family member or more than one covered family member that satisfies the remaining family deductible).

Once the entire family deductible is met by a combination of covered family members, you would be responsible for coinsurance until the family out-of-pocket is met.

    Buy-Up HSA Plan:

    • NON-EMBEDDED DEDUCTIBLE: For plans with a non-embedded deductible, if you have an individual coverage tier, once you satisfy the individual deductible then you pay coinsurance up to the individual out-of-pocket maximum. Once the individual out-of-pocket is reached, the plan pays at 100%.

For any tier other than individual (e.g., teammate & spouse, teammate & child(ren), or family), you and/or anyone in your family can satisfy the family deductible. For example, if you go into the hospital on day one and have a large hospital bill, you alone could satisfy the family deductible for the annual period. Once the family deductible is met by you and/or a combination of covered family members, then you would be responsible for coinsurance until the family out-of-pocket is met.

    Core HRA Plan:

    • The HRA can be used on all eligible out-of-pocket medical, prescription drug, dental and vision expenses.

    Base and Buy-Up HSA plans:

    • The Buy-Up HSA account will receive a contribution from DICK'S Sporting Goods, depending on the plan and tier of coverage elected.

    • Unlike an HRA, which is “employer-owned,” the HSA funds are “teammate-owned.”

    • The HSA accounts have pre-tax advantages and can be invested, like a 401(k), after achieving a balance of $1,000.

    Prescription Coverage

If you enroll in any of our medical plans, Evernorth Express Scripts will provide your prescription drug benefits. Evernorth Express Scripts will make sure you are getting the most effective and safe medications for your condition. They’ll contact you or your doctor if they see something that requires attention.

Consider the differences between current 2026 plans versus 2027 plans:

    Providers

Cigna is a medical carrier with a network of providers. When considering which medical plan is the best fit, be sure to check that your preferred providers are in the Cigna Open Access Plus (OAP) network. The relationship between you and your provider is an important one. You will want to select someone you feel comfortable discussing your health with, who has expertise in your area of need and is in-network with Cigna. Providers participating in the Cigna OAP network can be found in the Cigna Health Care Provider Directory.

    Spending Accounts

When you elect for medical benefits, DICK’S Sporting Goods will contribute the following into the Health Savings Accounts (HSAs) and Health Reimbursement Account (HRA) in 2027:

    • HSA associated with the HSA Base Plan: $250 Individual/$500 Family

    • HSA associated with the HSA Buy-Up Plan: $500 Individual/$1,000 Family

    • HRA associated with the HRA Plan: $250 Individual/$500 Family

What is a Health Savings Account (HSA) associated with HSA Base and HSA Buy-Up Plans?

Two of the medical plan offerings allow for an HSA. An HSA is a tax-advantaged savings account designed to help individuals save money for medical expenses.

If you enroll in the HRA plan, you will be able to use and direct your HRA funds for office visit copays, prescription drug copays, deductible expenses, coinsurance, and dental and vision expenses. You will determine how you want to use your HRA dollars. We also offer Health Care Flexible Spending Account (HCFSA) and Dependent Daycare Flexible Spending Account (DCFSA).

    • Does not apply to dental or vision coverage

    • No surcharge applies:

  • Supplemental Health and Wellbeing Programs

Supplemental Health - Accident, Critical Illness and Hospital Indemnity

You are eligible to enroll in Accident, Critical Illness and Hospital Care/Indemnity insurance benefits. These benefits are independent of medical insurance, so you can elect to have them with or without DICK’S medical coverage. If an accident occurs, you are diagnosed with a critical illness, or you are building a family, Supplemental Health insurance can be used toward medical bills, rent/mortgage payments, groceries or anything else. Supplemental Health insurance is excellent to have with the Core HRA Plan, Base HSA Plan or Buy-Up HSA Plan, which have higher deductibles than the Traditional PPO Plan.

Evernorth Express Scripts will make sure you’re getting the most effective and safe medications for your condition. They’ll contact you or your doctor if they see something that requires attention.

Here are some key features of your pharmacy benefits:

    Prescription Drug Changes

Brand medications move from copay to coinsurance. Mail order pricing changes to 3x retail and is no longer required.

Key items to note about your prescription drug coverage in 2027:

    • Brand Formulary and Brand Non-Formulary medications will move from a copay to coinsurance.

    • Mail order will no longer be required for maintenance medications.

    • You can fill up to a 90-day supply through a participating retail pharmacy.

    • The cost of a 90-day supply will change from 2x to 3x the 30-day supply cost.

Review your medical plan comparison to understand how these changes may affect your prescription drug costs.

    GLP-1s

Effective January 1, 2027, GLP-1 medications prescribed solely for weight loss will be excluded from coverage. The EncircleRx program will remain available for eligible teammates and dependents who are prescribed GLP-1 medications for diabetes management.

    Waste-Free Formulary

The Waste-Free Formulary helps keep prescription drug costs affordable by covering medications that provide the same clinical benefit at a lower cost when appropriate.

    The program:

    • Prioritizes safe, effective, lower-cost medication options

    • Helps reduce unnecessary prescription drug spending

    • Maintains the quality and value of your pharmacy benefit

If a medication change affects you, you'll be notified in advance and have access to:

    • Personalized support from a licensed pharmacist

    • Guidance on covered medication alternatives

    • Assistance understanding your pharmacy benefits and coverage

Support is available throughout the transition to help ensure you receive the medications you need.

    Generics

You always save more by asking your doctor to prescribe generic drugs. If no generic is available, ask your doctor to prescribe a brand-name drug on the formulary. To see if your medication is on the Evernorth Express Scripts formulary, visit Evernorth Express Scripts.

    90-day maintenance

You will need a 90-day prescription for maintenance medications. Your medication can be filled through mail order, or CVS or Walgreens pharmacies. For a three-month supply, you will pay three copays.

    Accredo

Accredo is the Evernorth Express Scripts Specialty Pharmacy and is your source for specialty prescription drugs. Be sure to let your doctor know that to receive your specialty medication, you must order through Accredo. If you have any questions, you can call Accredo toll free at 1-800-803-2523. Trained pharmacists and nurses are available 24/7 for any questions about your therapy.

    Drug Quantity Management Program

This prescription drug program is designed to make sure the quantity of prescription drugs you receive is considered safe. If you are prescribed a prescription drug that’s covered by this program, you will receive only the amount needed to last you a certain number of days. For instance, you would receive only 30 pills in one month for a medication that’s prescribed to be taken once a day.

    Prior Authorization Program

You need a prior authorization if prescribed a new medicine that is not on your formulary. To find out if your medicine requires a prior authorization, log in to express-scripts.com. Select “Price a Medication” from the drop-down menu under “Manage Prescriptions.” After you look up a medicine’s name, click “View coverage notes” or call the number on your member ID card. Your doctor can initiate a request for a prior authorization by calling Evernorth Express Scripts.

    Step Therapy Program

This program is designed for people who regularly take prescription drugs to treat ongoing medical conditions such as arthritis, asthma or high blood pressure. Step one medications are generic drugs that provide the same health benefits as higher-cost step two brand drugs. If a step one drug does not work for you, your doctor can request a prior authorization for a step two brand drug to determine if that will be covered by your plan. For more information on step therapy in your benefit plan, visit express-scripts.com or call the number on your member ID card.
